   Clyde Mark Collins, age 76, of North Ballard, died on February 8, 2015, at home following a forty year battle with Multiple Sclerosis.
He was born June 24, 1938, to T. Golden and Florell Marx Collins, near the Duchesne-Uintah county line across the highway from the present Union High School.  He attended elementary schools in Roosevelt, Ballard, and Alterra, graduating from Union High School with the class of 1956.  He later attended LDS Business College in Salt Lake City.
Clyde married Phyllis Irene Taylor September 24, 1959, in the Salt Lake LDS Temple.  
His chosen profession was the marketing and delivery of wholesale petroleum products, working with his father for fifteen years. He acquired ownership of the business in January 1970 and operated it until July 1979 when physical disability forced retirement.  He held the unique distinction of never living more than two and one-half miles from the house he was born in, and was never away from home for more than thirty consecutive days yet traveled 1.5 million miles within the boundaries of Duchesne, Uintah and Daggett counties.
His priorities in life were his wife of fifty-five years, his children, grandchildren, brothers, sisters, and other extended family members whom he enjoyed being with and visiting, and his faith in the Church of Jesus Christ of Latter-day Saints.  He served as ward clerk for more than 44 years with nine different bishops and as assistant stake clerk for four stake presidents.  He also enjoyed coin collecting, and helping his grandchildren assemble their coin collections.  In his younger years, he enjoyed fishing, hunting, and shooting.  He loaded many rounds of rifle, pistol, and shotgun ammunition for his family and friends.  
In the mid 1960s, he helped organize and was a charter member of Sheriff George Marret=s first Duchesne County Search and Rescue Jeep Patrol, where he served several years as secretary.  He participated in many searches and helped with the annual summer lamb and beef barbeque that raised funds for two way radios and other search and rescue equipment.  He helped coordinate intercounty relationships with other jeep patrols.  Again disability forced resignation.
Clyde enjoyed talking to old friends about growing up in Roosevelt and the changes made to Roosevelt and surrounding areas in the past 65 years.  
He is survived by his wife Irene; daughters Shanna (Delwyn) Palmer, Castle Rock, CO; Glenda (Duane) Huber, Yuba City, CA; sons, Steven (Susan) Collins, Troy (Judi) Collins, both of Roosevelt; and Gordon (Julie) Collins, Hyrum.  Brothers: Boyd Collins, Salt Lake City; Tom (Margie) Collins, Ogden; sisters, Mary Taylor, Orem; Gaye Berg, Gooding, ID; Kaye (Duane) Frisby, Payson; 26 grandchildren, 9 great-grandchildren; numerous cousins, nieces, and nephews.  
He was preceded in death by his parents, parents-in-law, Chuck and Pearlis Taylor, sister, Myrtle Cook, brother Allen Collins; brothers-in-law, Orlan Cook and Earl Taylor, sister-in-law, Gloria Collins, all his aunts, uncles, and several cousins.  
Funeral services will be held on Monday, February 16, 2015, at 11:00 a.m., at the Ballard LDS Chapel.  There will be a viewing on Sunday from 6-8 p.m. at the Hullinger Mortuary and from 10-10:45 a.m. Monday at the church.
Burial will be in the Lapoint Cemetery.
